SCHOLARSHIP PROGRAM APPLICATION
Application and all materials must be submitted/postmarked by April 3, 2017 in order to receive consideration.

Please upload a copy of your resume. Keep the resume to two (2) pages or less. Education, Experience and Memberships must be addressed by the resume.

Education:  List all post-secondary schools from which degrees were conferred. Official transcripts  from all post-secondary institutions which awarded Baccalaureate or other Master's degrees must be received by the deadline. Please include the name of the institution, dates attended, degree(s) and date awarded, and GPA.

Experience: Please list work, professional, business, military, internship, community, volunteer, etc. experience in the resume.

Memberships: List all honor societies, professional organizations, scholarships, prizes, honors, or class offices in the space below.

Please upload a statement of a maximum of 1,000 words describing your interests and work in your proposed field of study, including non-course educational experience, volunteer work, teaching or other relevant employment and publications. Indicate the specific competencies or characteristics you believe you offer the field of librarianship. This may include you career interests and goals, and your commitment to library services.
